Terms and conditions for All Access Plan (In-Person and Virtual)
FIT4MOM THOUSAND OAKS MONTHLY MEMBERSHIP AGREEMENT
This contract is made between the client and FIT4MOM Thousand Oaks and is a month to month agreement (after the
initial 3-month commitment) and except as provided in the initial 10-day cancellation policy below, may be
terminated by either party with the proper advance notice. There will be an early cancel fee of $297 plus any promotion received if
cancelled before the 3-month commitment. This membership allows you to attend unlimited Stroller
Strides/Stroller Barre, Body Well , Body Ignite& Strides 360 classes. If this plan was purchased on a promotional rate, the 3 month requirement begins once billing of $99 starts.
AUTHORIZATION FOR PAYMENT: I authorize FIT4MOM Thousand Oaks to initiate a charge to the account entered into Pike 13 for my dues. This authorization is to remain effective until I have cancelled my membership in writing through the FIT4MOM Thousand Oaks administrator. I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Thousand Oaks to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.
CANCELLATION POLICY To cancel this membership, the member must contact the FIT4MOM Thousand Oaks administrator at thousandoaks@fit4mom.com at least 30 days prior to their next payment due date to request a cancellation. Cancellations will be confirmed in writing to the member once they have been processed. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the Thousand Oaks franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged half the normal registration fee. Instructors are not authorized to cancel memberships.
HOLD POLICY Memberships may be held a maximum of 3 months per hold period. To place a hold on this membership, the member must contact the FIT4MOM Thousand Oaks administrator at thousandoaks@fit4mom.com at least 10 business days prior to their next payment due date to request a hold be placed on their membership. Holds will be confirmed in writing to the member once they have been processed. Hold starts will coincide with the next date dues is scheduled to be drawn out on and can end on any day of the month. When a hold period is ended, the monthly auto payments will automatically resume on the return date. It is up to the client to keep track of their return date. There will be a $20 processing fee charged to your credit card on file for the hold period which will be credited back upon your return bill. Instructors are not authorized to place a hold on memberships.
10-DAY CANCELLATION POLICY You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM Thousand Oaks within the initial ten days. You are responsible for paying for the classes already attended. FIT4MOM ON-DEMAND MONTHLY SERVICE
With your paid monthly membership you are given free access to FIT4MOM OnDemand. You will have unlimited access to OnDemand workouts. When your FIT4MOM membership is properly cancelled according to the Terms and Conditions, your OnDemand subscription will be automatically cancelled. If you would like to keep your OnDemand subscription you can re-enroll using the same login information and purchase your desired paid subscription.
